Note: Course approval need be obtained ONLY if the staff member expects to submit the course for tuition reimbursement in which case the approval of the Superintendent must be obtained, in writing, prior to the start of course. When course is completed you must send your GRADE and proof of payment for courses taken. Once the $1,000 reimbursement cap is met for a given year, it is not necessary to send other applications for approval. All documents must be submitted to the personnel office no later than September 15th for reimbursement in October.
Note: Complete the form on-line. Then print and sign the form. Return it to the Payroll Department in the Administration Building.
Note: Use this form to submit articles for the newspaper to Publicity Team Members. All form must be submitted in electronic format.
